Before scheduling an appointment, it is important to have a referral for each discipline, that comes from the primary physician and a picture of the insurance card as the team members needs to validate it.
If you have United Healthcare Community plan, the doctor must give you an authorization done through the United Healthcare portal for each of the evaluations needed.
